REFERRALS

Referrals are not required to book an appointment with Yasmina Diab.

However GP Chronic Condition Management Plan (formally known as Chronic Disease Management Plans) and Eating Disorder Plans are welcomed.

If your GP has written a plan for you, please have them send it through to admin@diabdietetics.com.au prior to your consultation.

EATING DISORDER PLANS

If you are eligible for an Eating Disorder Plan (EDP), your plan will need to be written by your usual GP.

Having an Eating Disorder Plan allows you to received a Medicare rebate for 20 sessions.

To be able to use a EDP for your appointment the plan will need to be in-date, at the time of your consultation to be able to claim it through Medicare.

GP CHRONIC CONDITION MANAGEMENT PLAN REFERRALS

Formally known as “Chronic Disease Management Plans or CDM”

If you are eligible for a GP Chronic Condition Management Plan (GPCCMP), your plan will need to be written by your usual GP.

Having a GP Chronic Condition Management Plan allows you to received a Medicare rebate up to 5 sessions if you choose all 5 visits to be allocated to a dietitian provided.

To be able to use a GPCCMP for your appointment the plan will need to be in-date, at the time of your consultation to be able to claim it through Medicare.

Thursdays 9.30 am to 5.00 pm

Friday 9.30 am to 5.00 pm

Appointment Lengths

Initial Consult = 60 minutes

Standard follow up = 60 minutes

Short follow up = 30 minutes

Cancelling or Changing Appointments

You can always change your face-to-face appointment to a telehealth, on the same day, by emailing us at admin@diabdietetics.com.au.

As admin support is only available on Fridays, to cancel or change an appointment please email admin@diabdietetics.com.au.

*Please be aware that we have a 24 hour cancellation policy, fees will apply for an appointment cancelled within 24 hours of your scheduled appointment time*

Medicare and Private Health Rebates 

For patients utilising an Eating Disorder Plan or GP Chronic Condition Management Plan, Medicare claims are processed on Fridays.

You should receive your rebates in your bank account by the Monday following your appointment.

If you do not receive your rebate within this timeframe, please email us.

For patients claiming via private health cover or an NDIS plan, your receipts will now be emailed to you on Fridays to facilitate your claims.

Please note that administrative support is available on Fridays only, 9:00am to 5:00pm.
