Referrals

Written referrals are not required for consultations however, if you do have a
referral, you or your Doctor can email it to admin@diabdietetics.com.au.

Eating Disorder Plans & Chronic Disease Management Plans are accepted and there is
a cost associated with these plans.

Consultations are face to face in clinic
or via telehealth.

  • Referrals are not required to book an appointment with Yasmina Diab.

    However she does accept GP Chronic Condition Management Plan (formally known as Chronic Disease Management Plans) and Eating Disorder Plans.

    If your GP has written a plan for you, please have them send it through to admin@diabdietetics.com.au prior to your consultation.

  • If you are eligible for an Eating Disorder Plan (EDP), your plan will need to be written by your usual GP.

    Having an Eating Disorder Plan allows you to received a Medicare rebate for 20 sessions.

    To be able to use a EDP for your appointment the plan will need to be in-date, at the time of your consultation to be able to claim it through Medicare.

  • Formally known as “Chronic Disease Management Plans or CDM”

    If you are eligible for a GP Chronic Condition Management Plan (GPCCMP) your plan will need to be written by your usual GP.

    Having a GP Chronic Condition Management Plan allows you to received a Medicare rebate up to 5 sessions if you choose all 5 visits to be allocated to a dietitian provided.

    To be able to use a GPCCDM for your appointment the plan will need to be in-date, at the time of your consultation to be able to claim it through Medicare.

  • You can always change your face-to-face appointment to a telehealth, on the same day, by emailing us at admin@diabdietetics.com.au.

    As admin support is only available on Fridays, to cancel or change an appointment please email admin@diabdietetics.com.au.

    *Please be aware that we have a 24 hour cancellation/dna policy, fees will apply for an appointment cancelled within 24 hours of your scheduled appointment time*

    You will receive your appointment reminder 48 hours in advance, allowing you plenty of time to cancel or reschedule before any fees apply.
